NEW ORLEANS, LA (WVUE) - The New Orleans Police Department is looking for a juvenile reported missing from a health facility in Algiers.

The incident happened on Monday around 9:45 a.m., according to NOPD.

Jaquan Singleton 17, was last seen leaving the Provisions Seaside healthcare facility in the 6300 block of Stratford Place after a disagreement with the staff members, according to the report.

Singleton has not been seen nor heard from since that time.

He is described as being approximately 5'5" and 120 pounds.  He was last seen wearing black jeans, a black jacket, a gray and red bulls ball cap and black and white tennis shoes. He was last seen carrying a black backpack, according to NOPD.

The report said Jaquan suffers from impulse control and conduct disorder and has only stayed at this facility since Friday.
