BOURNE, MA (WBZ/CNN) – Officials in a Massachusetts town are seeking answers after a 2-year-old reportedly lost a toe when she hit a crack on a broken playground slide.

Witness Lindsey Simmons says she couldn’t believe what she saw Tuesday at the Buzzard’s Bay splash pad and playground in Bourne, MA.

First responders rushed to help a toddler who allegedly lost a toe on one of the playground's slides.

"I had heard the little girl cry, but I thought that she didn't want to leave. So, I had no idea," she said.

Simmons says her own son went down the slide before and after that little girl.

"A woman tapped me on the shoulder and said, 'Excuse me, we need to – someone's going to come over to look at the slide because there's a toe in there,'" Simmons said.

Since the incident, the park has been closed, with caution tape wrapped around the slide. A bottom piece of the playground structure has been removed.

Town Administrator Thomas Guerino has called a meeting Thursday to determine how the 2-year-old sustained her injuries. He says he’s trying to get answers.

The slide had been on a list for repairs in the new park that just opened Memorial Day weekend.

"If engineers had told us to close it, it would have been closed," Guerino said.

Some parkgoers say the slide had been blocked off by caution tape prior to the incident, but Simmons says there was no caution tape when the girl was injured.

The toddler was treated at a hospital in Rhode Island. Her condition is unknown.
